DIRECTIONS FOR COMPLETING THE INVENTORY AND APPRAISAL FORM

Important Note: Inventory and appraisement must be filed with the clerk of the Probate Division within thirty days from date of letters, unless longer time is granted. Continuance requests must contain a reason.

The law requires that the inventory contain a reasonably detailed description of all property of the decedent or protectee, listing fair market values of all real and personal property as of the date of death or date appointing conservator, together with a statement of all encumbrances, liens and other charges on any item. The inventory must be typewritten or electronically produced. It does not have to be filed on the Probate Court form, but may be created on a computer, typewriter, or word processor, provided it contains substantially the same format, language, classifications of property and the affidavit of the personal representative or conservator as the court form, and is compliant with 473.233, 473.237 and 475.145 RSMo.

  1. REAL PROPERTY. As to decedents, list plat or survey descriptions, book and page numbers, and street addressees of Missouri real estate only. As to protectees, list this information on all real estate. List the amount of any mortgages or liens against any parcels of real estate.
  2. FURNITURE, HOUSEHOLD GOODS AND WEARING APPAREL. No detailed listing or appraisal of items is required except as to those items specifically bequeathed.
  3. CORPORATE STOCKS. List number of shares, names of companies, classes of stock, and fair market values as of date of death or date appointing conservator. Also indicate whether shares are listed on a securities exchange, sold over the counter or closely held. Listing of certificate numbers is not required.
  4. BONDS, NOTES AND OTHER DEBTS OWED TO THE DECEDENT OR DISABLED PERSON. List face values, names of companies or debtors, the maturity dates, rates of interest, the amounts of interest accrued at date of death or date appointing conservator, the names of pledgees, if any, and any recording data. Listing of certificate numbers is not required. Treasury bonds may be itemized by listing face values, series classification, issue dates, and values or they may be grouped by denominations. Certificate numbers are not required.
  5. BANK ACCOUNTS, INSURANCE, MONEY. As to bank accounts, list names of banks, account numbers, and account balances; as to insurance, list names of insurance companies, policy numbers, face values, and amount of benefits payable, and for protectees list cash surrender value and any beneficiaries.
  6. JUDGMENTS AND PROCEEDS OF LAWSUITS. Specifically indicate the Cause No., the court in which it was rendered, and the amount recovered on behalf of the decedent or protectee. A copy of the Judgment or Settlement Agreement signed by the judge must be attached.
  7. ALL OTHER PERSONAL PROPERTY OWNED BY DECEDENT OR PROTECTEE, INCLUDING PROPORTIONATE SHARES IN ANY PARTNERSHIP. Describe vehicles fully, listing year, name of manufacturer, model, and vehicle identification number. No detailed list of partnership assets is required but the ownership percentage of the partnership should be included, with a value. (see 473.220 RSMo. re: partnership inventories)
  8. PROPERTY POSSESSED BUT NOT OWNED BY THE DECEDENT AT DEATH OR BY PROTECTEE. List such property, together with a statement as to the knowledge of the personal representative or conservator as to its ownership.
  9. ADDITIONAL INFORMATION IN CONSERVATORSHIPS. The full value and description of all jointly-held property of the protectee must be included, together with a statement of all periodic income or benefits payable to the protectee.
