Massage Definition | Application Process

Any business or person operating in the unincorporated area of St. Louis County doing physical massage either in a regular location or as an outcall (calls received for services to be conducted in the unincorporated area of St. Louis County) requires a Massage Establishment or a Massage Outcall License.

Submit the completed appropriate application (Sole Owner) along with the initial license fee of $500.00 and all documents requested in the application.

Instructions For Massage Establishment License

(New Applicants Only)

The information listed below must be submitted with the completed application and signature. The application must be notarized. Applications will be sent to the Police, Health, and Public Works Departments for their review and approval. Please note the pending location cannot be within 1,000 feet from the property line of a church, school, (licensed daycare) or synagogue.

The following must be submitted:

  • A check or money order in the amount of $500.00 for the initial application, $100.00 for renewal.
  • A Copy of the Registration of Fictitious Name with the Secretary of State of Missouri, if necessary.
  • Recent photo of the front of the premises.
  • Bill of sale (if purchased from another owner).
  • Copy of the lease or rental agreement. If you own the property, submit proof of ownership.
  • Drawing or diagram of the premises.
  • Copy of the Certification of Incorporation and Articles of Incorporation, if filing as a corporation.

Each owner, partner, corporate officer, director, and stockholder shall submit:

  • Two 2" x 2" photos of self.
  • Legal document proving date of birth (valid driver's license, birth certificate, etc.).

Each masseur, masseuse, and employee must furnish:

  • Two 2" x 2" photos of self.
  • Legal document proving individual is eighteen (18) years of age.
  • Written notification of violations of criminal statues or ordinances (other than minor traffic violations).
  • Written proof of training and experience in the field of massage.
  • Copy of written agreement, if independent contractor or consultant, etc.

(Renewals Only)

If application is a renewal submit the following:

  • Completed application, signed and notarized.
  • Letter from doctor stating applicant is free from communicable diseases.
  • Renewal fee of $100.
